Foxwoods announced that the requirements for the Wellness program will be waived again this year, which means there will be no penalty for members that do not fulfill the three requirements:

•Getting a physical by 9/30/21

•Biometric form filled out by your physician and returned to Foxwoods by 9/30/21

•Calling the Wellness Coach: 1-800-352-5071 by 9/30/21

If you do fulfill all three by 9/30/2021 you will get the Wellness Rebate

There will not be a wellness coach at the casino, so you have to call the number above. You can pick up the biometric form at the HR office.
